Care Coordination for Children (CC4C) is a program which provides formal case management to eligible children from birth to five years of age, who are at risk for poor health outcomes. This program is offered FREE OF CHARGE to residents of Alleghany, Ashe and Watauga counties.
Who is eligible?
- Children identified with special health care needs or at risk for physical, behavioral or emotional conditions
- Children that require health related services above and beyond what is generally required
- Children in foster care families
- Children with chronic medical conditions
- Children who are in short or long term stressful situations
The goal of CC4C is to work with families to identify specialized support services for themselves and their children. Through this program families will have increased access to services and children with special needs will have maximum opportunity to reach their developmental potential.
Staffing includes registered nurses or social workers who provide information regarding needed preventive and specialized services for children and their families. This program is family-centered and family-driven and seeks to respond to the varying concerns of children and their families recognizing that:
- concerns of children and families change over time
- families are the constant in the lives of children
- families have expertise regarding their children
What can a CC4C manager do to support families and children?
- Provide information about a wide variety of family-oriented resources
- Discuss family strengths and concerns through home visits, telephone calls and other personal contacts
- Identify programs, services, and resources that meet your family needs
- Provide developmental screenings
- Serve as a link between you and your child’s doctor or nurse
- Suggest ways you can strengthen parent-child relationships
- Introduce you to parent support programs when available
- Offer encouragement and support
